Seaside Health Plan members have access to a variety of services, some of which require additional documentation. Please see below for links to forms and documents you may need.

Continuity of Care

If a member’s current healthcare provider is leaving the Seaside Health Plan network, they may be eligible for continuity of care if they have certain qualifying conditions. If a new member within Seaside Health Plan is currently receiving active treatment from a provider who is not within the network, they may also qualify for Continuity of Care. Please refer to the Continuity of Care Program policies and if the member qualifies, they may submit an application.
